To make a referral, or arrange an appointment with any of our therapists, please contact Crisalida or download a referral/registration form and email/fax/send to our Intake Co-ordinator.  Weekly team meetings are held to determine the most appropriate treatment plan/therapist, so it may take a week for a response.
