Crisalida prides itself on providing a truly individualised service for each client. If after checking above to ensure the service you’re after is open, please call or email our reception team with the client’s name, date of birth and an email address so that we can send you an intake form.

After receiving the completed form, requests for services are considered in the context of client age, needs and client/therapist availability. Our intake team meets regularly to determine the appropriate referral pathway within our centre and will contact you after review to provide options and probable wait times (if any).
